I mentioned a few months ago that my On & Off Task

What is it? A quick form for observing and calculating how much a student appears to be on and off task during a lesson, etc.
How do I use it? I keep a bunch of these in my file drawer and grab them whenever I need them. These are my “go to” when a teacher refers a child to the intervention process for attention because they allow me to get a quick glimpse of the student and provide some strategies based on what I see.
