I mentioned a few months ago that my On & Off Task behavior observations are the most popular request I get from general education staff. I think it’s the colorful graphics! This is a great tool to use with general education students and for special education evaluations as well!

What is it?  A quick form for observing and calculating how much a student appears to be on and off task during a lesson, etc.

How do I use it?  I keep a bunch of these in my file drawer and grab them whenever I need them. These are my “go to” when a teacher refers a child to the intervention process for attention because they allow me to get a quick glimpse of the student and provide some strategies based on what I see.

Create it: Whatever you feel like the behaviors you need to address most in observations are, create a template based on what you use most.
